This is an exciting opportunity to be involved in a Test and Learn Health & Housing Coordinator service pilot scheme to support safe, appropriate and sustainable hospital discharges that meet the needs of individual patients. To support, reduce delayed and premature discharges, reduce unscheduled admissions, readmissions and A+E attendances.
You will work closely with hospital discharge teams to provide an effective, timely and robust approach to patient discharge. Build relationships and work closely with HBC colleagues, other agencies and the voluntary sector to secure more rapid access to expertise to achieve best results for patients.
Issues to be resolved may include:
- Homelessness or risk of homelessness
- Housing issues relating to self-neglect and/or hoarding
- Repairs / Minor Adaptations / DFG and speed of access
- Supporting with access to housing related benefits, grants and loans
- Support with accessing more suitable accommodation
- Warm and healthy homes
Peer support will be available from other Health & Housing Coordinators across the Test and Learn pilot.
This post is co-located at NHS hospitals and at the Council Offices.
This post is 4 days a week, there is scope to discuss and agree a working pattern to meet both the needs of the service and the post holder.
