Landlord Questionnaire
The questionnaire has 7 steps which contain requests for information as set out below.
Step1 of 7 Company details
Questions in this section relate to the following:
- The company Contact details.
- The company registered address.
- The company numbers
- Questions about whether the company is not for profit, a Registered Social Landlord (RSL), a Voluntary Organisation a charity or other type of organisation
- If your company is not an RSL, details about whether your company has applied to be one or was formerly one.
Step 2 of 7 Supported accommodation resident details
Questions in this section relate to the following:
- The address of the property
- The number of units in the property and what type of units they are.
- The client group that the property will cater for
- The age range of the client groups
- The name of all tenants if there are any.
Step 3 of 7 Ownership and lease information
Questions in this section relate to the following:
- Are there any links between the owner of the property or the organisation that you lease the property from and your organisation?
- Do you receive any help financially from the owner of the property or the organisation that you lease the property from?
- If the property or units within it are empty, do you still pay rent for the lease.
- Copies of lease documents
Step 4 of 7 – care, support, and supervision details
Questions in this section relate to the following:
- The care providers contact details.
- Whether you take part in accreditation or benchmarking
- How you decided on the rents
- What working relationships and or links your organisation has with Hyndburn
- Details of your referral process
- How you think the premises meet your tenants needs
- How the CSS needs of your tenants are determined
- Information on the type of support that is provided, by whom is it provided, for how long, how often, and how it is funded?
- How often is the CSS requirement reviewed?
- What are the tenants perceived outcomes?
- If the tenants are not from Hyndburn, we would require reasons as to why they have been placed in Hyndburn.
Step 5 of 7 Tenant Liability – Rent, Costs and Charges
Questions in this section relate to the following:
- A full breakdown of the rent is required with detailed, quantified, evidenced breakdowns of the Intense Housing Management costs, service charges and care & support costs.
- Details of how the core rent was determined.
- Details of the rooms available to the tenant solely or shared,
Step 6 of 7 –Tenant Liability – Rent, Costs and Charges
This section allows you to provide further information that you feel is pertinent.
Step 7 of 7 – Disclaimer and declaration – Rent, Costs and Charges
This section contains a disclaimer and has a declaration to sign.
NB – all sections, questions, and document uploads are mandatory (except instep 6) and dependent on how questions are answered more details may be requested on the form and for proof may be required to be uploaded.
